A Message From Dr. Susan Blue
For over twenty-five years it has been the commitment of our practice to offer excellent and compassionate care to patients with a wide range of neurological disorders. Some of the conditions that we evaluate and treat include:

MAKE AN APPOINTMENT
Your primary physician will often decide if you should see a neurologist. He will arrange any necessary referrals required by your insurance plan. If you think yourself that you need to see a neurologist, contact us and tell us briefly about your symptoms. We cannot express specific medical opinions about patients that have not yet been evaluated in the office, but we can perhaps decide if an appointment is necessary, or if you need another type of specialist.
If you are scheduling your first consultation, you can fill out the Patient Business Information Form and Health Information Form in our Downloadable Forms section (above). The forms are detailed but it is necessary for us to have broad knowledge of your medical and family history. Please bring a physical copy of your radiology images.
