Whether you are a new patient or an existing patient, we have to have extensive background information and updated information for your medical and accounting records. Please print out all of the New Patient Forms, complete them and bring them with you at the time of your appointment. We have to have this information before we can see you. If your insurance changes before your appointment, please let us know immediately, well before your appointment.
You can also complete the forms electronically and the mail them to us before your appointment or bring them with you at the time of your appointment. We cannot accept them electronically since they have Protected Health Information.

For return/active patients (you have been seen in the office within the past twelve months), you can print out the Update form below and bring it with you at the time of your appointment. It is very important for us to have your current insurance information at least ten workdays before your appointment if possible. If you need a referral to come to our office, please be sure you have requested it and it has been processed. If you have difficulty getting a referral or you are told to call our office about a referral, please contact us at Extension 12.
Since we do not allow cell phone use in the office, if you have any electronic images, lab work, or medical information that you want to present, please print it out and bring it with you in printed form to your appointment.
